Step 1: Confirm Your Service Area

To ensure we can provide you with care, new patients must live within our service area. Please enter your information below to confirm your location. If you are within our service area, DFD’s new patient forms will be available for immediate download.

Step 2: Complete Your New Patient Forms

Please download* and complete DFD’s new patient forms. When you’re finished, you can upload them securely using the form below or mail them to our office.

Step 3: Schedule Your First Appointment

Once your paperwork is processed, we’ll schedule your first visit. Please note that it can take up to six weeks to complete this process and get your first appointment on the calendar.
