We’re excited that you’re considering joining the DFD community! Here’s everything you need to know about becoming a new patient.

Step 1: Confirm Your Service Area

To ensure we can provide you with care, new patients must live within our service area. Please see the map at the bottom of this page to determine if you reside within our service area.

Step 2: Call Us

Give us a call at 207-524-3501. Our care team will let you know if we’re accepting new patients and, if so, will guide you through the next steps and answer any questions.

Step 3: Complete Your New Patient Forms

We’ll mail your new patient paperwork directly to your home. You’ll have 30 days to fill out and return these forms.

Step 4: Schedule Your First Appointment

Once your paperwork is processed, we’ll schedule your first visit. Please note that it can take up to six weeks to complete this process and get your first appointment on the calendar.
